A form of declaration signed and issued by a nominated person on charge of the work to be carried out on or near to overhead traction wiring equipment or high voltage apparatus. the purpose of the form is to make known to the person in charge of the work exactly which equipment is isolated and upon which, or near to which, it is safe for the work to be carried out so far as the respective equipment is concerned.
