Case plan

    An instrument which identifies assessed needs and individual program directives for an offender to address while under supervision of the doc. in juvenile corrections, a case plan is an individualized service plan that targets a juvenile’s areas of risk, need and prepares him/her for progressively increased responsibility and independence in the community.
